Why was my H1-B only granted for one year instead of three?

INS is currently using interim rules until the final health care certification process is defined. As part of these temporary provisions, H1-B petitions are only being approved for one year. However, we're still getting a few mysterious three year approvals, so apparently not everyone at INS has heard.
